Biblical Guidance Meeting Request

This form is to be filled out by any person within Casa de Alabanza who wishes to meet with the Pastoral office for Biblical Guidance.

Communications Request Form

This form is to be filled out by department head leaders to request any type of media such as Emails, Announcements, Social Media, Printed Materials, Shirts, or any Communications for Casa de Alabanza. Please submit this form 60 days prior to your due date.

Event/ Meeting Request Form

This form is to be filled out by department head leaders to request any type of event/ gathering at or on behalf of CDA. Please make sure to submit this form at least 60 days prior to the event or 30 days prior to a meeting for approval. Once approved by the pastoral office you may submit the requests on Planning Center Resources and/ or submit a communications request form if necessary.
